  • Line 3: Address Provide your permanent residential address. It should be the place where you live, not a temporary address or a mailing address.

  • Line 4: Mailing Address (if different) If you have a separate mailing address (e.g., a P.O. Box), enter it here. Otherwise, leave this line blank.

  • Line 5: U.S. Tax Identification Number (TIN) If you have a U.S. TIN, such as a Social Security number (SSN), Individual Taxpayer Identification Number (ITIN), or Employer Identification Number (EIN), enter it. If not, leave this line blank.

 

3. Certification

By signing Form W-8BEN, you certify that the information provided is accurate and that you are the beneficial owner of the income. You also confirm that you are not a U.S. person.

 

4. Additional Notes

  • Date of Birth: Although not required, providing your date of birth can help prevent confusion.

  • Foreign Tax Identification Number (TIN): Some countries issue TINs. If you have one, include it in the appropriate section.

5. Submitting the Form

Important Information

If a change in circumstances makes any information on the Form W-8BEN you have submitted incorrect, you must notify the withholding agent, payer, or FFI with which you hold an account within 30 days of the change in circumstances and you must file a new Form W-8BEN or other appropriate form. Furthermore, if you become a U.S. citizen or resident alien, you must notify the withholding agent, payer, or FFI within 30 days of becoming a U.S. citizen or resident alien.
